Where can I order engraved brass tags for my plaques in Utah

I'm in the Bountiful/Salt Lake area and am looking for about 25-30 custom brass (or whatever metal is common for this sort of thing, I don't know) tags for my dads antler plaques that just say something like his name, the year, and the state or area the animal was harvested in..

Example:

John Smith - 2009
Fish Lake, Utah

Or something to that extent. Just to be drilled onto each plaque so 30 or 50 years from now I'll know it and be able to see them all when their in the cabin or a garage or something.

Anyone know where I can have some of these made up? I'm only home in this area for a week, then its back to work and I know my dad doesn't care about this so it's something I'm going to hopefully find someone to do somewhat quickly (at least have the order put in and then next time I'm home they'll be here and I can just screw them on).
